Abstract
Fistulas of the larynx or pharynx occur in the course of treatment by surgery or radiation of cancer in the oral cavity or throat.

More often they are complications of surgery after irradiation already has been tried in attempt to eradicate the primary cancer.

Recently, they can be induced as the results of increasing traffic accidents or industrial casualties.

Inability to swallow, the constant eflux of salivary secretions, the hazard of aspiration these secretions when a tracheostomy is present make this a most troublesome if not ubearable condition.

Proper reconstruction of this type of defect should aim toward the restoration of a leak-proof wall having a sufficient lining tissue to permit swallowing and esophageal speech when the larynx has been removed.

Author repaired the defect of anterior hypopharyngeal wall associated with laryngel box fracture due to car accident using local inverted flap and epaulette flap in one stage, and report this case with the review of literatures.
